Needle Holders

What are needle holders?

Needle holders grip a curved suture needle at the jaws and lock it there while it is driven through tissue. The three handle designs suit different working styles: ring handles with a finger-ratchet, Mathieu-style spring handles that lock and release with a squeeze, and Castroviejo spring handles held like a pen for microsurgical control. Tungsten carbide inserts give a harder, replaceable gripping surface and hold a needle longer than plain steel jaws.

How the needle holders range is organised

Spring handle, palm grip:
Mathieu, Mathieu 6.25", Mathieu with narrow serrated jaws and carbide inserts.
Microsurgical, pen grip:
Castroviejo straight and curved 5.5", micro Castroviejo smooth tip curved, micro Jacobson smooth tip curved.
Ring handle:
Crile Wood 6" and Crile Wood 6" TC, Mayo Hegar, Derf, Webster, Collier 5", Boynton 5".
Combination and specialty:
Olsen Hegar with scissors, Neivert with one offset ring, Baumgartner, Gardner.

Choosing the right needle holder

Match the jaw to the needle. Fine intraoral suturing on 4-0 to 6-0 wants Castroviejo or Derf; anything heavier wants Crile Wood or Mayo Hegar. The Olsen Hegar combines holder and scissors in one instrument, which saves an instrument change but cuts suture accidentally if you are not used to it. Carbide inserts are worth it on the holder you reach for every day — they resist the jaw wear that makes an older holder let a needle rotate.
